Care Anywhere: Kaiser Permanente’s Approach in Prioritizing Care Across the Continuum

Kaiser Permanente runs the largest integrated not-for-profit health system in the United States and sets a benchmark on care across the health care continuum. However, recent health care changes in the U.S. created even more opportunities to serve their patients who required more home and community support. Keynote speaker Trish Rodriguez is a Senior Vice President responsible for leading the seamless integration of care from acute to community in one of Kaiser Permanente’s largest service areas.
 
The implementation of Ontario Health Teams provides a similar opportunity for Ontario’s health system. Trish will discuss how Kaiser Permanente’s journey applies here and will share guidance on how providers can move forward in collaboration. There will be valuable insights for all health system partners on how health and social care can synergize in an integrated health system.

Senior Vice President and Area Manager
Kaiser Permanente South Sacramento Area

Trish Rodriguez is the senior vice president and area manager for Kaiser Permanente in South Sacramento and Elk Grove. She oversees integrated health plan and hospital operations, which includes a Level II trauma center. She is responsible for the provision of health care to more than 247,000 Kaiser Permanente members in the South Sacramento County area. Kaiser Permanente insures over 50 percent of the South Sacramento County population and employs approximately 3,400 staff members and nearly 400 physicians.

​Trish is a health care professional with more than 28 years of broad experience in health care operations and strategic planning in a variety of leadership positions. She has been with Kaiser Permanente for more than 26 years. Her career spans from Alberta, Canada to Hawaii to California. Trish is a graduate of the Harvard Business School Executive Leadership Program, completed her master's in public health at the University of Hawaii, and holds a bachelor's in nursing from the University of Lethbridge in Alberta, Canada.

Cynthia Banks is a dynamic story teller who has written, produced and directed memorable, international, award-winning television documentary for CBC, TVOntario, Discovery and NFB.  Banks’ documentaries are amongst some of the top-rated shows in Canadian prime time documentary programming.

The Caregivers' Club: A Documentary Director's Personal Journey.

Following filmmaker Cynthia Banks’ personal experience caring for her father who had Alzheimer disease, Banks wanted to profile the crisis within the healthcare system, but turned her attention instead to the true heroes, the at-home caregivers who are the foundation of care.  

Her presentation, including excerpts of the Canadian Screen Award nominated film, The Caregivers’ Club, explores some of the systematic and emotional challenges for family caregivers but also reveals and reflects on the lessons she learned about herself during her personal caregiving journey.  The film is dedicated to her mother, Phyllis Banks, loyal caregiver to her father, Donald.

Building A Digital Health Playbook for Home and Community Support - NEW!

The Ministry of Health recently launched the Ontario Digital Health Playbook, a plan to help the Ontario Health Teams build their digital health plan and capacity. Join future strategist Zayna Khayat as she considers what a digital health playbook integrating home and community could look like.  Zayna will discuss new technologies that will be used in home and community support in the future, the impact on how we will interact with clients, and what it will take to be truly connected and integrated to the health care system.

Zayna Khayat, Future Strategist with SE Health

Zayna Khayat is a Future Strategist with SE Health (formerly, Saint Elizabeth Health Care) a health and social impact enterprise with a major focus on keeping older adults healthy in their homes. Zayna is Faculty of Singularity University’ Exponential Medicine stream, and is adjunct faculty in the Health Sector Strategy stream at the Rotman School of Management at the University of Toronto.

Dr. Khayat was previously the lead of health system innovation at MaRS Discovery District, a 
health innovation hub in Toronto, Canada. In 2017
 she was seconded to the REshape Innovation Centre at Radboud university medical centre in the Netherlands. Zayna completed her Ph.D. in diabetes research from the University of Toronto (2001), followed by a career in strategy consulting, including as a Principal in the healthcare practice of The Boston Consulting Group (BCG). She currently resides in Toronto with her husband and three children.

Linda Davis, President & CEO, Northumberland Hills Hospital
Linda joined Northumberland Hills Hospital as President and Chief Executive Officer in November 2013. A senior executive with over 30 years of experience in the Canadian health-care sector, she was previously CEO of the Collingwood General & Marine Hospital, a role she held for nine years.

A Registered Nurse, she trained at the University of Alberta Hospital in Edmonton. Her career has taken her from that province to Vancouver, British Columbia and Montreal, Quebec, where she obtained a Bachelor of Science degree in nursing and a Master’s of Science, both from McGill University, before moving in 1990 to Ontario.

Throughout her career, Linda has held senior leadership positions in a number of hospitals. She has also worked within the private sector as a health care consultant

Linda is passionate about engaging staff and physicians with the focus of continually improving the quality and safety of patient care.

​Raechelle Devereaux, ED, Guelph CHC
Raechelle is the Executive Director at the Guelph Community Health Centre, where she has worked for just under a decade.

Throughout her career, she has championed the development of comprehensive integrated care models that improve the health and well-being of in community, with a focus on addressing the social determinants of health for complex populations.

The Guelph CHC is an active "core" partner in the Guelph and Area Ontario Health Team, where Raechelle is working alongside health leaders to transform care for community residents, with primary care as a key foundation of the developing system.


​Ashnoor Rahim, VP, Community Care, WoodGreen
Ashnoor oversees WoodGreen’s Community Care Unit which is responsible for Assisted Living & Seniors Programs and Mental Health, Addictions and Developmental Services Programs. Through this portfolio, Ashnoor is leading the work on Seniors Crisis services for the City of Toronto and is the co-lead of the Seniors Help Line. In addition, she is the executive lead for the Community Access and Navigation Program (CNAP). Ashnoor also serves as the Community Lead for both the East and Mid East sub regions.
​
WoodGreen Community Services is an anchor partner for East Toronto Health Partners (full submission status), and Downtown East Toronto (in development status), as both groups continue to apply to become an Ontario Health Team (OHT). This leaves Ashnoor with an in-depth knowledge of the OHT process in the Toronto area.

Ontario Health Teams: Round 1 Lessons Learned Panel

For 31 groups of health service providers, the past few months have been dedicated to filling out the full Ontario Health Team application form. Come listen to a panel from every corner of the province discuss their challenges and lessons learned of going through the process first.

Panelists include Linda Davis, President and CEO Northumberland Hills Hospital, Raechelle Devereaux, Executive Director, Guelph Community Health Centre and Ashnoor Rahim, Vice President, Community Care, WoodGreen Community Services .
