2017 OCSA Commitment to Care Awards

Celebrating the exceptional accomplishments of Ontario's not-for-profit home care and community support providers.


​For the third year, OCSA is pleased to host our Commitment to Care Awards, presented by Lough Barnes Consulting Group. These awards are designed to shine a well-deserved spotlight on the dedication, skills and creativity of our members. Winners will be chosen by a selection committee of health system experts and leaders, and recognized on October 18th, during the evening banquet during our annual conference.

OCSA 25th Anniversary Leadership Award
​

Awarded to an individual who has demonstrated exceptional leadership and dedication to the home and community support sector over the course of 25 years of service.
​

OCSA Award for Commitment to Quality Improvement
​

Awarded to an organization that demonstrates excellence and innovation in service delivery and performance measurement.

OCSA Award for Emerging Leadership
​

Awarded to an individual early in their career who has demonstrated creative leadership and made a positive impact on the home and community support sector.

OCSA Award for Commitment to Exceptional Client and Family Care

Awarded to an inspiring individual or team with a powerful story about going above and beyond to make a meaningful impact in the lives of clients and their families.

OCSA Award for Commitment to Volunteer Service

Awarded to an outstanding individual who has made significant, sustained and unpaid contributions to the home care and community support sector.  

Eligibility & Considerations

Eligibility
  • Nominees in all categories must be current staff or volunteers with OCSA Members in good standing. In addition:
    • For OCSA 25th Anniversary Leadership Award: Nominees must have a cumulative total of 25 years of service to the home and community support sector, in any combination of roles, including as staff, volunteers, or board members. Roles need not be consecutive.
    • For OCSA Award for Emerging Leadership: Nominees must be emerging leaders in the home and community support sector, having worked in the sector for fewer than 5 consecutive years. They should hold mid-level positions (managers, coordinators, supervisors) and have led within the scope of those positions, with potential to continue to improve the lives of clients and caregivers in the future.
    • If you would like to nominate a leader who has between 5 and 25 years of service, please consider doing so in the Commitment to Exceptional Client and Family Care category.
 
  • Award winners must be able to attend the awards ceremony during the OCSA Conference banquet on October 18, 2017.
  • Nominee must be willing to provide short bio and photographs for use in OCSA and award sponsor communications. Winners will be asked to provide additional materials for use during the awards ceremony, and to deliver a brief acceptance speech.
  • Self-nominations are permitted. Only one nomination form per organization or individual, per category, please. OCSA reserves the right to combine or omit duplicate nominations.
  • OCSA board members are not eligible for individual awards.​
  • Winners from previous years are not eligible to win again in the same category. However, they may be nominated for a different award category.

Nomination Process
  • Award nomination process is available online only. Click here to submit. You'll be asked to provide your name and e-mail to quickly create a free account on our submission platform in order to access the nomination forms.
  • Nominations must be received by August 10, 2017.
  • Winners will be notified the week of September 18, 2017.

Considerations
  • Award winners will be announced during the OCSA Conference banquet.
  • Submission must not contain confidential client information or restricted intellectual property (IP) that should not be circulated in the public domain.
  • The information contained in the submission may be used by OCSA for promotional purposes. This may include imagery contained within the application.
  • Applicants are expected to grant OCSA consent to publish any material contained in the application.

Winner Recognition
  • Award winners will be announced during the OCSA Conference banquet on October 18, 2017.
  • Award winners will receive one complimentary registration for 2018 OCSA conference. 

​Selection Criteria
  • Please review the selection criteria and rubric before completing your nomination. 
  • Ensure that you use concrete evidence and examples to demonstrate your answers whenever possible – this will improve your score!
